Dr. Anna Woo of the University of Toronto and colleagues were enrolled in a retrospective study of 61 patients with ICD who were 46 years old (including primary prevention of sudden cardiac death, cardiac arrest or secondary prevention of persistent ventricular tachycardia).
After an average follow-up of 40 months, 8 patients developed an appropriate discharge. Secondary prevention patients (36%) were more frequent than primary prevention (8%). A total of 20 cases (33%) of inappropriate ICD discharge occurred. Two patients developed an inappropriate and appropriate discharge. Inappropriate ICD discharge is associated with a history of atrial fibrillation, leading to fractures. Less than 30 years old (hazard ratio 3.0) and previous history of atrial fibrillation (hazard ratio 3.1) are significant predictors of abnormal discharge.
